Leeds Dying Well in the Community Project is using a whole systems approach to support effective service redesign for community based palliative and end of life care in Leeds. The project which began in 2020 continues to work across the City to make the best use of the resources available. Its ultimate aim is to ensure the delivery of effective and compassionate care outside of hospital for those people who are dying, and their carers and families.
