Welcome to LPCN News October 2022

Welcome to the autumn edition of LPCN News. This year is passing quickly, and we know that many of you are busy preparing for the challenging winter months ahead.

We have been busy working closely with the Palliative and End of Life Care Population Board to agree the new processes and governance structure around our work.

Thanks as always for your support and we will keep you posted. We are also progressing with lots of exciting projects across the City, please read on for more updates.

Bereaved Carers Report – sharing the learning across Leeds

Our Leeds Bereaved Carers Survey report is now online! The survey provides important insights into how carers feel about the experience of end of their life care services in Leeds.

Huge thanks to everyone involved and the team at Healthwatch Leeds for their hard work and help with the survey and putting the report together. We are currently looking at the recommendations and we will keep you up to date with the plan of action. View the full report here.

Equality and Diversity Group Plan – now published!  

We have recently published the LPCN Equality and Diversity Group plan for year one. As a network we are committed to helping the City of Leeds strive towards improving access to palliative and end of life care for all people from all the communities we serve regardless of their background.

The Group will focus on making impactful and meaningful change as soon as possible. View the plan here.

Dying Well in the Community Update

The Dying Well in the Community projects are continuing to progress, with the team now focusing on potential Single Point of Access (SPOC) arrangements for palliative and end of life care, to ensure people entering the system can do so in a simple and responsive way.

The Seacroft project team are collaborating to see how they might further integrate and improve end of life care offered across their Local Care Partnership area

Timely Response Project Launched

We have funded a new piece of work called the Timely Response project, which aims to identify people coming towards the end of their life in a timely way, in order to offer support and enable them to help plan their care in advance.

This will be trialled in Leeds Central Primary Care Network and the findings will inform the plan for across the City.

Communications Update

The Communications Group met in August to discuss further developing the website and other key LPCN documents. We will keep you up to date with progress and if you have any feedback on the website, please let us know! www.leedspalliativecare.org.uk

End of Life Respiratory Group

A new End of Life Respiratory Group met for the first time this month to review how the services work together to offer the most appropriate support, to consider information provided to patients and what education might be required for the clinicians involved in providing the care.

 

Respect Audit

We have recently funded a project to audit ReSPECT in Leeds, to gain insight how we might improve engagement and information. The project will be led by colleagues from the
University of Leeds.
