Welcome to LPCN News June 2022

Welcome to the Summer edition of LPCN news. We are pleased to announce that our Annual Report is now published, summing up our progress and achievements over the last financial year.

We are also delighted to be embarking on several new exciting work programmes, from Health Inequalities monitoring to funding several new local projects. Read on for more news.

Annual Report – now live!

Our Annual Report for 2021/22 is now available on our website here   
We’d like to thank all our partners for their ongoing support and commitment to all LPCN work and projects. As we move forward, we will continue to refine and extend our collaborative, system-wide approach which is essential to meeting the demographic, resource and clinical challenges to excellent care for all.

 

Successful bids to kickstart LPCN projects
 
We have recently been awarded Health Inequalities funding for two key projects. One to continue to further develop and improve the Homelessness service. The other to commence Sexual Orientation and Gender Identity Monitoring for people in the LGBTQ+ population accessing palliative and end of life care, in line with If We’re Not Counted, We Don’t Count national guidance.

Our new Equality, Diversity and Inclusion (EDI) Group will oversee and support these projects. Watch this space for further updates.

LPCN supports local projects
 
We are pleased to say LPCN have recently approved several business cases from partners and have agreed to fund:

  • Promotional materials for the LTHT SUPPORT Campaign  in July. This will allow the Palliative Care Team to engage with staff and spread the word across the Trust about how they can support relatives and friends of people who are receiving end of life care whilst in hospital.
     
  • Clinicians and community partners to undertake a pilot looking at collaborative working to improve patient experience in the Seacroft Local Care Partnership area. 
     
  • ECHO Training for the newly appointed LPCN Clinical Educator and Education Administrator. 

Leeds Dying Well in the Community Update
 
Our Dying Well in the Community project continues to make progress and has entered Phase 2. Key areas of focus include the scoping out of what a Single Point of Access for palliative and end of life care might look like in Leeds. We are also working on improving integration in the Seacroft Local Care Partnership area to see if we can collectively improve the service offer to patients and their families.

A short survey has been sent to over 100 contributors to gain their feedback on the Whole Systems Approach work led by Leeds Beckett University. A copy of the final phase one report can be found here
